Delivered by air from the Martin Baltimore plant to the 21st BG at MacDill Field, Tampa Bay, Florida (from 24/4/43), where it was used for 391st BG phase one training. Crashed at MacDill Field, Tampa, Florida at 0944 hours EWT on 29/5/43, returning from a review flight as a three ship formation, killing all six of the crew. The aircraft flown by 2nd Lt. Elmer D Martin, crashed on approach to land at MacDill Field to refuel before returning to home base at Myrtle Beach, South Carolina. All three aircraft extended their landing gear at the same time whilst turning on the final approach. The first aircraft was cleared to land, but the second ship about 200 feet above the other aircraft was ordered to go round. This aircraft flown by 2nd Lt. Elmer D Martin, the third to land encountered severe prop-wash from the first aircraft, and was seen to enter a vertical bank to the left. The pilot levelled, out but then the aircraft began to yaw to the left, and then to the right, before it entered a half roll to the right and crashed into the ground in an inverted attitude, exploding in flames upon impact. Listed as a 391st BG / 574th BS crew in training. The final entry on the aircraft record card lists, "MacDill awaiting survey per gain and loss report of 6/6/43". Note, there is no mention on the aircraft record card that this aircraft was converted to a B-26B-20, and just refers to a B-26B-1 model.
